This Privacy Policy (“Privacy Policy”) describes how Alluvium Health, Inc. and its affiliates
and subsidiaries (collectively, “Alluvium,” “we,” or “us”) collect, use, disclose and otherwise
process information about you. This Privacy Policy applies to information we collect when you
access or use our website (collectively, the “Alluvium Services” or “Services”), or when you
otherwise interact with us, such as through our customer support channels or our social media
pages. We encourage you to read this Privacy Policy carefully. Terms not defined in this Privacy
Policy shall have the meaning set forth in Alluvium’s Terms of Use.
To the extent that you are an end user, customer, or patient of one of our Business Customers
(“End User”), please contact the Business Customer directly with any questions regarding the
processing of your information. This Privacy Policy does not apply to our processing of End
User’s information on behalf of our Business Customers.
1. Notice of Privacy Policy Changes.
Alluvium may update this Privacy Policy to reflect changes to information practices. If we make
changes, we will notify you by revising the Effective Date at the top of this Privacy Policy. If we
make any material retroactive changes, we will provide you with additional notice (such as by
adding a statement to the Alluvium Services or sending you a notification). You are encouraged
to review this Privacy Policy upon each visit to the Alluvium Services to stay informed about our
information practices and the choices available to you.
2. Collection of Information
The information we collect about you depends on how you interact with us. In this section, we
describe the categories of information we collect and the sources of this information.
Information You Provide to Us.
Alluvium collects information from visitors to our Alluvium Services, (“Website User(s)”) as
well as when you create an account, make a purchase, fill out a form, sign up for marketing
communications from us, request a demo or customer support, or otherwise communicate with
us as a business customer of the Alluvium Services (“Business Customer”) or Website User.
You can be both a Business Customer and Website User.
Such information includes, but is not limited to:
- Name;
- Telephone number;
- Job title and company name;
- Passwords and log-in credentials; and
- Any other information that you choose to provide.
Information We Collect Automatically
We automatically collect information about your interactions with us or our Services, including:
- Device, Usage, and Activity Information: We collect information about how you access
our Services, including data about the device and network you use, such as your hardware
model, operating system version, mobile network, IP address, browser type, and unique
device identifiers. We also collect information about the route by which you access our
Services, access dates and times, and your activity on our Services, such as pages viewed and
links clicked. - Audio Recordings: If you contact customer service by phone, we may monitor and retain
those conversations, including for training purposes. - Information Collected by Cookies and Similar Tracking Technologies: We use tracking
technologies, such as cookies and pixels, to collect information about your interactions with
the Services and our marketing communications. These technologies help us improve our
Alluvium Services and marketing communications, personalize your experience, and analyze
your interactions with us, including to see which areas and features of our Alluvium Services

Information We Collect from Other Sources
We obtain information from other sources. For example, we may collect information from

- Provide, maintain, and improve the Alluvium Services;

- Personalize your experience with us;
- Send you technical notices, security alerts, support messages, and other transactional or
relationship messages; - Send you marketing communications (see the Communications section below for information
about how to opt out of these communications at any time); - Monitor and analyze trends, usage, and activities in connection with our products and
services; - Target advertisements to you on third-party platforms and websites (for more information
and to opt out, see the Targeted Advertising and Analytics section below); - Detect, investigate, respond to, prosecute, and help protect against security incidents and
other malicious, deceptive, fraudulent, or illegal activity, and help protect the rights and
property of Alluvium and others, including to enforce the Alluvium Terms of Use and
Acceptable Use Policy; - Comply with our legal and financial obligations;
- To perform functions as otherwise described to you at the time of collection; and
- To create de-identified and aggregate data that cannot reasonably be linked to you.
You should not disclose medical information when communicating with Alluvium customer
support, either by email, by phone, or by any other means. Any medical information you disclose
to Alluvium customer support is not subject to federal or state medical privacy regulations.
4. Targeted Advertising and Analytics
We engage others to provide analytics services, serve advertisements, and perform related
services across the web and in mobile applications. To do this, Alluvium and these partners may
use cookies, pixels, SDKs, and similar technologies to collect information about your use of our
Services and other websites and mobile apps, including your device identifiers, IP address, web
browser and mobile network information, pages viewed, time spent on pages, links clicked, order
information, and conversion information. This information is used to deliver advertising targeted
to you on other companies’ websites and mobile apps, understand the effectiveness of this
advertising, analyze and track data, determine the popularity of certain content, and better
understand your activity.
You can also learn more about interest-based ads, or opt out of having your web browsing
information used for behavioral advertising purposes by companies that participate in the Digital
Advertising Alliance, by visiting www.aboutads.info/choices. Your device may also include a
feature that allows you to opt out of having certain information collected through mobile apps
used for behavioral advertising purposes.
5. Your Choices.
You can choose not to provide certain information, but failing to provide certain information
may result in you being unable to use certain features of the Alluvium Services. You may also
correct, delete inaccuracies, change or remove any information from your account by contacting
Alluvium at support@alluviumhealth.com, provided, however, that Alluvium may retain
certain information as provided or permitted by law.
Additionally, you may choose to reject “cookies” or disable mobile device identifiers, as
described above. You can usually adjust your browser settings to remove or reject browser
cookies. While you are not required to accept cookies or mobile device identifiers from
Alluvium, if you block or reject them you may not have access to all Alluvium Services or this
will affect some of the functionality of the Alluvium Services.
6. Disclosure of Information
Alluvium will disclose information to third parties only in the ways and for the purposes that are
described in this Privacy Policy (including as described in the Targeted Advertising and
Analytics section above and the below scenarios). Alluvium may disclose information:
(a) to third parties that provide services on behalf of Alluvium and/or in connection
with the Alluvium Services, including to our vendors, service providers, contractors and
consultants who perform services on our behalf, such as companies that assist us with web
hosting, payment processing, financing, fraud prevention, customer service, analytics, and
marketing;
(b) in response to a request for information from law enforcement authorities and
individuals involved in legal proceedings if we believe that disclosure is required or permitted
pursuant to any applicable law, regulation, court or administrative order, or legal process,
including lawful requests by public authorities to meet national security or law enforcement
requirements;
(c) if we believe that your actions are in connection with your violation of or to
otherwise enforce Alluvium's rights under the Terms of Use, Acceptable Use Policy, or other
agreements with Alluvium;
(d) as reasonably required to protect the business of Alluvium;
(e) if we believe it is necessary to protect the personal safety and/or rights of
employees of Alluvium, our Business Customers, or members of the public;
(f) our legal, financial, insurance, and other professional advisors where necessary to
obtain advice or otherwise protect and manage our business interests;
(g) between and among Alluvium and our parents, affiliates, subsidiaries, and other
companies;
(h) with your consent or at your direction, as we make personal information available
to third parties when we have your consent or you intentionally direct us to do so.
Further, Alluvium may be required to disclose your information to third parties: (a) for the
satisfaction of any debt or upon liquidation of Alluvium should such a situation arise; and (b) in
connection with, or during negotiations with, a purchaser or transferee of all or a portion of
Alluvium's assets in conjunction with the sale or transfer of such assets, or a merger, financing,
or acquisition of all or a portion of our business by another company; provided, however, that in
the case of (a) and (c), the transferee will either assume this Privacy Policy or establish security
measures and privacy practices no less secure than those set forth herein.
We also disclose aggregated or de-identified information that cannot reasonably be used to
identify you.
7. Information Retention and Removal.
We store personal information associated with your account for as long as your account remains
active. Alluvium may retain information for legally permitted purposes and as set forth in this Privacy Policy, for the period of time reasonably necessary for such purpose. If required by law,
Alluvium will delete your information by erasing it from Alluvium's database. Alluvium will
also respond to written requests from you to delete any of your information that is eligible for
deletion in accordance with Alluvium's then-current policies or as required under applicable law.
However, Alluvium will retain and use your information as necessary to comply with legal
obligations, resolve disputes, and enforce Alluvium's Terms of Use and or other contractual
obligations.
8. Third Party Websites.
The Alluvium Services may contain third party advertising and links to third party websites.
Alluvium is not responsible for the privacy practices or policies of these advertisers or websites,
and does not manage or control such practices. Please contact such advertisers or websites
directly for more information about their privacy practices. Alluvium expressly disclaims, and
each User expressly releases Alluvium from, any and all liability whatsoever for any
controversies, claims, suits, injuries and/or damages, arising from and/or in any way related to
third party advertisers and/or the use of links to third party websites that may appear on the
Alluvium Services, including without limitation such websites's availability, privacy policies,
content, advertising, and/or products or other materials.
9. Communications
You may, in any commercial e-mail sent by Alluvium, opt-out of receiving such
communications in the future using the link at the bottom of the email, although we may still
send you non-promotional emails, such as those about your account.
Users should not provide any information to Alluvium regarding any individual residing in the
European Union or the European Economic Area, and by using the Alluvium Services, the User
agrees that it will not provide such information and the provision of such information will be a
material violation of this Privacy Policy and the Terms of Use.
